Social Worker 
(Full-time - 40 hours)

 

 

POSITION SUMMARY:

 

The Medical Day Care Social Worker performs psychosocial assessments on assigned child/family, and participates in the development of the interdisciplinary care plan. He/she orients child/family to the program and care plan while facilitating optimal family involvement in program.  Provides education regarding disability, as well as facilitates utilization of community resources to meet the child and families identified needs.  In addition, is responsible for home visits, supportive counseling/support groups, and discharge planning services.

Job Requirements

POSITION QUALIFICATIONS:

 

Master Degree in social work from an accredited graduate School of Social Work

At least one year of full time supervisory experience in a licensed health care facility

Excellent communication and collaboration skills

Active New Jersey license

 

PHYSICAL DEMANDS/WORKING CONDITIONS:

 

Predominantly:  Moderate Work as defined by the Dictionary of Occupational Titles (DOT). 

  Lifting 50 pounds maximum and occasionally lifting and/or carrying objects. Walking, bending, and standing are often necessary in carrying out job duties.  The position characteristics described are representative of those an employee encounters while performing the essential functions of this job.  However, reasonable accommodations may be made to enable qualified individuals with disabilities to perform the essential functions.

 

Tasks assigned to this position may involve potential and/or direct exposure to blood, body fluids, infectious disease, air contaminants, and hazardous chemicals
