Responsibilities for Telephonic Care Coordinators include: Engaging members telephonically to coordinate services, community resources, and treatment needs Delivering a holistic approach to coordinated care based on the enrollee needs Identifying early risk factors and conducting ongoing assessments and documenting in an electronic file Creating, reviewing and revising Plan of Care plans and focusing on disease management Functioning as an advocate for members and decreasing unnecessary hospitalizations Collaborating with the members PCP to deliver and coordinate necessary services Building relationships with members and their families; assisting them with proper health choices Ensuring cases are documented in a timely manner Generally work is self-directed and not prescribedWorks with less structured, more complex issues Serves as a resource to othersUnitedHealthcare Community & State is part of the family of companies that make UnitedHealth Group one of the leaders across most major segments of the US health care system. QualificationsMasters level degree in Social Work or other related field Mustbe a licensedsocial worker (LSW)with an active and unrestricted license in the State of New Jersey. Minimum of 1 year clinical or case management experience Proficiency in PC-based word processing and database documentation (Word, Excel, Internet, Outlook) Ability to travel in the local market up to 50 miles if needed(reliable and insured transportation) Ability to complete 4+ weeks training in Newark, NJ office (non-reimbursable) Ability to multitaskAdditional Assets Preferred: Minimum of 1 year experience in long-term care, home health, hospice, public health, or assisted living Medicare/Medicaid experience A background working in geriatric special needs Experience with electronic charting Bilingual in any language a plusDiversity creates a healthier atmosphere: equal opportunity employer M/F/D/VUnitedHealth Group is a drug-free workplace.
