RN - INTAKE COORDINATOR:Mercer Home Health Care whose office is located in Bordentown, NJ, a leading provider of skilled home healthcare service throughout Mercer County is seeking an experienced RN - Intake Coordinator to join our team. This exciting full time position will interact with the entire home care team and is responsible to coordinate and process home care referrals from hospitals, skilled nursing centers, physician offices and other sources.
The ideal candidate must have a current NJ RN License; will also have home healthcare, admissions director/coordinator, or equivalent experience, a knowledge of Medicare and managed care plans, a background in healthcare, and exceptional customer service and phone skills. The proper candidate may be an admissions coordinator at a skilled nursing center, a current home care Intake Coordinator or related hospital position.
