Expanding throughout New Jersey, Advoserv of New Jersey is currently in search of a dedicated professional to fill the position of Nurse Case Manager. Our Nurse Case Managers are valuable members of our Medical team, report directly to the Nurse Manager, and responsible for coordinating and overseeing the medical care of all residents.
Located in the Swedesboro area of Southern New Jersey, this is an excellent opportunity for qualified job seekers entering the human services field as well as those looking to continue their career with a growing organization committed to quality, compassionate care. Please note, this is a full time position and part time positions are not currently available.
Work Hours:
- Full time
- Monday thru Friday
- Rotating on-call responsibilities
- Other hours as necessary due to program needs
- Requires travel to program locations in Gloucester and Salem counties
Essential Functions & Responsibilities Of This Full Time Position:
- Ensure the health and welfare of all residents
- Monitoring the general health of all residents
- Coordinating the medical care for all residents
- Assisting with the administration of medications
- Linking and coordinating services
- Assess resident medical needs, determine appropriate medical treatment and initiate the treatment process
- Carrying out physicians’ orders as instructed
- Assist in maintaining emergency medical files for residents
- Maintain open and ongoing communication with all other staff and administrators
- Participate in administrative meetings and conduct in-services as necessary
- Participate in the IHP process for all Individuals by providing medical information and contributing to goal development
- Available for on-call coverage to ensure 24 hour nursing availability to the program

Job Requirements
The ideal candidate will have:
-
Maintain active New Jersey RN or LPN licensure without discipline against license or privilege to practice
One year of experience in the nursing field
Experience working with children or adults with intellectual/developmental disabilities &/or mental health diagnoses preferred

Strong written and oral communication skills

Sucessful at juggling multiple tasks, follow through, managing details, being organized and meeting deadlines

Ability to work independently, self-motivated and goal oriented
-
Proficiency in relationship building & advocacy
-
Strong desire to work with neurologically-challenged individuals
-
Proficiency in Microsoft Office including Word, Outlook and Excel
-
Valid Driver’s License without restrictions and without any major violations within the past three years
- Successful completion of AdvoServs pre-employment screening process which includes fingerprinting for state and national background checks, motor vehicle record check, physical exam including drug screen, and tb (tuberculosis/mantoux) screening
