We are now hiring a full time OT at our SNF Van Dyk Manor of Ridgewood, NJ
The primary purpose of the position is providing and directing occupational therapy services to residents, including assessment, treatment, program planning and implementation, related documentation, and communication. Functions under physician???s orders.

UTIES
& R
ESPONSIBILITIES
Provides occupational therapy services to residents and initiates referrals when appropriate and in accordance with Occupational Therapy Department Procedures.
Conducts screenings of residents at regular intervals to determine need for intervention/treatment.
Evaluates residents to obtain data necessary for treatment planning and implementation. Conducts specialized evaluations as indicated.
Interprets and communicates evaluation findings to residents, families, and team members.
Establishes individual resident schedules in conjunction with other team members.
Develops treatment plans by identifying a target list, including short and long-term goals and methods to achieve identified goals.
Coordinates treatment plan with resident, family, and other team members.
Implements and conducts treatment as outlined in treatment plan.
Monitors resident???s response to intervention and modifies treatment as indicated to attain goals in accordance with physician orders.
Develops appropriate home or programming within the community to maintain and enhance the performance of the resident in their own environment.
Documents results of resident???s assessment, treatment, follow-up, and termination of services.
Adapts treatment to meet the individual needs of the resident.
Provides, recommends, fabricates adaptive devices, or other equipment. Trains resident, family, and appropriate staff in the use or application of orthotics and prosthetics.
Participates in discharge planning.
Supervises and directs Certified Occupational Therapy Assistant (COTA) in the implementation of the treatment plan.
Supervises Rehabilitation Aides, students, and volunteers.
Reviews the quality and appropriateness of the total services delivered and individual occupational therapy programs for effectiveness and efficiency, using pre-determined criteria.
Participates in resident, family and staff education.
Confers formally and informally with other team members in coordinating the total rehabilitation program of the resident.
Adheres to Occupational Therapy Department procedures regarding documentation of occupational therapy services.
Provides in-service education to the members of the resident???s care team and education to the community.
Provides treatment in accordance with established standards of practice, department procedures and productivity standards.
Consistently demonstrates sound judgment in the evaluation, planning implementation and follow-up of resident therapy programs.
Reports to work on time and coordinates schedule to achieve maximum productivity and efficiency during assigned shift.
Assists with and participates in Quality Assurance Program as required.
Maintains a current knowledge of law and regulations regarding rehabilitative services and consults with supervisor to ensure that all employment actions are in compliance with applicable regulations.

Ensures a safe environment and utilizes tools and equipment in a safe manner complying with all infection control, universal precautions, and OSHA standards for the healthcare professional.
Reports all hazardous conditions including incidents, injuries and equipment to Supervisor/appropriate personnel immediately.
Responds to and acts appropriately in emergency or disaster situations.
Adheres to high standards of cleanliness, grooming, hygiene and dress code.
Adheres to all policies of company, including job description, certification requirements, mission statement, HIPAA, confidentiality standards, and resident rights.
Adapts to changes in responsibilities in a positive manner, is receptive to new ways of completing tasks, and is flexible in carrying out assignments.
Communicates and interacts professionally and effectively with residents and coworkers throughout the community ensuring a cooperative environment while maintaining self-control.

DUCATION
:
Bachelor of Science in Occupational Therapy from an accredited program.
Successful completion of National Certification Examination for OTR.
Assumes responsibility for ongoing continuing education and professional development.

ICENSURE
Current state licensure or license eligible.
