Registered Wound Treatment Nurse at Livingston

The Wound Treatment Nurse is responsible for the day to day coordination and oversight of all aspects of the Wound Management Program. Primary function- Perform weekly skin assessment and wound rounds with members of the interdisciplinary team assesses and document the appearance of the wound, stage the wound and measure all areas, noting progress or lack thereof.  Ensure notification to the physician of all identified concerns.  Know and utilize company approved skin and wound care products and treatment protocols.     Reporting to:
  • Director of Nursing Services
  Supervising:
  • Work in conjunction with the Unit Manager and Charge Nurse for all shifts.
  Essential Duties and Responsibilities:
  • Confer with Director of Nursing Services and/or other licensed nurses regarding skin and wound disorders of residents.
  • Consult with the Care Planning Team concerning assessment evaluations and assist in planning and developing the skin and wound care interventions for residents.
  • Review resident’s medical record and assess need for physician evaluation if indicated.
  • Complete required forms, reports, evaluations, charting, etc. per wound management program and protocols.
  • Implement and maintain established policies and procedures of the Wound/Skin Management Program.
  • Make written and oral reports/recommendations to the attending physician, Medical Director, DON and Dietary Consultant.
  • Agree not to disclose resident’s protected health information and promptly report suspected or known violations of such disclosure to the Administrator.
  • Report and known or suspected unauthorized attempt to access facility’s information system.
  • Serve on, participate in, and attend various committees as appointed by the Administrator and/or DON (i.e. Resident At Risk meeting).
  • Assess wounds and provide a weekly status report to the DON, UM, QA Nurse (as applicable), Dietary Manager and Dietary Consultant.
  • Maintain an effective, friendly working relationship with health professionals, physicians, consultants and family members involved.
  • Provide, within established protocols and under the supervision of the physician, treatment for wound and skin disorders as directed by the attending physician or Medical Director.
  • Ensure that residents with skin conditions and wounds receive appropriate prevention and treatment, such as daily inspection, turning and activity, appropriate dietary intervention and are clean and dry.  Report noncompliance to the UM and QA Nurse (as applicable).
  • Assist with as needed and/or monitor completion of admission and discharge skin assessments.
  • Discharge teaching of residents/family regarding skin and wound care as required.
  • Notify attending physician and RP of changes in wound/skin status as required and request a change in treatment orders as assessed to be necessary.
  • Participate as requested by Administrator/DON in the planning and conducting of in-service training classes concerning wound and skin policies, procedures and protocols.
  • Attend and participate in annual facility in-service training programs as scheduled and required.
  • Attend and participate in continuing education programs designed to keep you abreast of changes in your profession, as well as to maintain your license on a current status.
  • Assist in monitoring the inventory of medications, medical supplies and equipment to ensure and adequate supply of skin care products are on hand to meet the needs of residents.
  Job Requirements:   Education: 
  • Graduate of an approved RN program.
  Professional Experience:
  • One year LTC experience preferred.
  • Knowledge/experience in wound management preferred.
  Certification/Licensure:
  • Actively licensed as an RN in assigned state of employment.
